Background: Diabetes mellitus (DM) is not associated with increased mortality in critically ill patients, a phenomenon known as the "diabetes paradox". However, DM is a risk factor for increased mortality in patients with COVID-19. This study aims to investigate the association of DM and stress-induced hyperglycemia at intensive care unit (ICU) with mortality in this population. Methods: This is a retrospective study. Electronic medical records from patients admitted from March 2020 to September 2020 were reviewed. Primary outcome was mortality. Secondary outcomes were ICU and hospital mortality and stay, and need for mechanical ventilation and renal replacement therapy. Results: 187 patients were included. Overall mortality was 43.2%, higher in patients with DM (55.7% vs. 34%; p = 0.007), even after adjustment for age, hypertension, and disease severity. When patients were separated into groups, named normoglycemia (without DM and glycemia ≤140 mg/dL), stress-induced hyperglycemia (without DM and glycemia >140 mg/dL), and DM (previous diagnosis or HbA1c ≥ 6.5%), the mortality rate was 25.8%, 37.3%, and 55.7%, respectively (p = 0.021). Mortality was higher in patients with higher glycemic variability. No statistical difference related to secondary outcomes was observed. Conclusions: DM, hyperglycemia, and glycemic variability associated with increased mortality in critically ill patients with severe COVID-19, but did not increase the rates of other clinical outcomes. More than stress-induced hyperglycemia, DM was associated with mortality.

During the nineteenth century, a major change took place in the trade, production, and use of mercury that altered its nearly exclusive link to silver refining in the Hispanic New World. We track the global expansion of mercury markets in chronological detail from 1511 to 1900 using historical archives on production and trade, a detailed country-by-country accounting of the pool of anthropogenic mercury from which legacy mercury was ultimately generated. The nature and profile of pre-1900 legacy mercury extends beyond silver refining, mercury production, and gold extraction, and includes alternate sources (vermilion, felt, mercury fulminate) and new regions that were not major silver or gold producers (China, India, United Kingdom, France, among others), that accounted for approximately 50% of total mercury consumed in the nineteenth century. The nature of the pre-1900 mercury market requires a quantitative distinction between legacy mercury and historic anthropogenic mercury production and use, since the chemistry of its end-uses determines the pathways and timelines for its incorporation into the global biogeochemical cycle. We thus introduce the concept of a mercury source pool to account for total historic anthropogenic mercury within and outside this cycle. Together with a critical review of previous assumptions used to reconstruct the historical use and loss of mercury, a much lower level of emissions of pre-1900 legacy mercury is proposed. A coordinated effort across disciplines is needed, to complete a historically accurate scenario that can guide the multilateral policies adopted under the United Nations Minamata Convention to control mercury in the environment.

Recent studies demonstrate a short 3-6-month atmospheric lifetime for mercury (Hg). This implies Hg emissions are predominantly deposited within the same hemisphere in which they are emitted, thus placing increasing importance on considering Hg sources, sinks and impacts from a hemispheric perspective. In the absence of comprehensive Hg data from the Southern Hemisphere (SH), estimates and inventories for the SH have been drawn from data collected in the NH, with the assumption that the NH data are broadly applicable. In this paper, we centre the uniqueness of the SH in the context of natural biogeochemical Hg cycling, with focus on the midlatitudes and tropics. Due to its uniqueness, Antarctica warrants an exclusive review of its contribution to the biogeochemical cycling of Hg and is therefore excluded from this review. We identify and describe five key natural differences between the hemispheres that affect the biogeochemical cycling of Hg: biome heterogeneity, vegetation type, ocean area, methylation hotspot zones and occurence of volcanic activities. We review the current state of knowledge of SH Hg cycling within the context of each difference, as well as the key gaps that impede our understanding of natural Hg cycling in the SH. The differences demonstrate the limitations in using NH data to infer Hg processes and emissions in the SH.

Environmental mercury (Hg) contamination is a global concern requiring action at national scales. Scientific understanding and regulatory policies are underpinned by global extrapolation of Northern Hemisphere Hg data, despite historical, political, and socioeconomic differences between the hemispheres that impact Hg sources and sinks. In this paper, we explore the primary anthropogenic perturbations to Hg emission and mobilization processes that differ between hemispheres and synthesize current understanding of the implications for Hg cycling. In the Southern Hemisphere (SH), lower historical production of Hg and other metals implies lower present-day legacy emissions, but the extent of the difference remains uncertain. More use of fire and higher deforestation rates drive re-mobilization of terrestrial Hg, while also removing vegetation that would otherwise provide a sink for atmospheric Hg. Prevalent Hg use in artisanal and small-scale gold mining is a dominant source of Hg inputs to the environment in tropical regions. Meanwhile, coal-fired power stations continue to be a significant Hg emission source and industrial production of non-ferrous metals is a large and growing contributor. Major uncertainties remain, hindering scientific understanding and effective policy formulation, and we argue for an urgent need to prioritize research activities in under-sampled regions of the SH.

The present study prepared, optimized, and characterized solid lipid microparticles that contained trans-anethole (SLMAN), evaluated their antiinflammatory activity in acute and chronic inflammation models, and investigated their effects on the gastric mucosa in arthritic rats. The microparticles were obtained by a hot homogenization process and characterized by physicochemical analyses. The acute inflammatory response was induced by an intradermal injection of 0.1 ml of carrageenan solution (200 µg) in the hind paw. The rats were treated orally with a single dose of SLMAN 1 h before induction of the inflammatory response. The chronic inflammatory response was induced by the subcutaneous application of 0.1 ml of complete Freund's adjuvant suspension (500 µg) in the hind paw. SLMAN was orally administered, starting on the day of arthritis induction, and continued for 21 days. The results showed that SLMAN was obtained with good encapsulation efficiency. Treatment with SLMAN at doses of 25 and 50 mg/kg was as effective as trans-anethole (AN) at a dose of 250 mg/kg on acute and chronic inflammatory responses. Histological analyses showed that treatment with SLMAN did not aggravate lesions in the gastric mucosa in arthritic rats. These results indicated that treatment with SLMAN at a dose that was 5-10 times lower than non-encapsulated AN exerted an inhibitory effect on acute and chronic inflammatory responses, suggesting the better bioavailability and efficacy of microencapsulated AN without aggravating lesions in the gastric mucosa in arthritic rats.

Abstract Objectives The Pain Catastrophizing Scale-Child version (PCS-C) allows to identify children who are prone to catastrophic thinking. We aimed to adapt the Brazilian version of PCS-C (BPCS-C) to examine scale psychometric properties and factorial structure in children with and without chronic pain. Also, we assessed its correlation with salivary levels of Brain-Derived Neurotrophic factor (BDNF). Methods The Brazilian version of PCS-C was modified to adjust it for 7-12 years old children. To assess psychometric properties, 100 children (44 with chronic pain from a tertiary hospital and 56 healthy children from a public school) answered the BPCS-C, the visual analogue pain scale, and questions about pain interference in daily activities. We also collected a salivary sample to measure BDNF. Results We observed good internal consistency (Cronbach's value = 0.81). Parallel analysis retained 2 factors. Confirmatory factor analysis of our 2-factor model revealed consistent goodness-of-fit (IFI = 0.946) when compared to other models. There was no correlation between visual analogue pain scale and the total BPCS-C score; however, there was an association between pain catastrophizing and difficulty in doing physical activities in school (p= 0.01). BPCS-C total scores were not different between groups. We found a marginal association with BPCS-C (r= 0.27, p= 0.01) and salivary BDNF levels. Discussion BPCS-C is a valid instrument with consistent psychometric properties. The revised 2-dimension proposed can be used for this population. Children catastrophism is well correlated with physical limitation, but the absence of BPCS-C score differences between groups highlights the necessity of a better understanding about catastrophic thinking in children.

RESUMEN

Objetivo: analisar a produção nacional existente sobre educação e relações interprofissionais. Metodologia: constitui-se um estudo bibliográfico, semelhante à revisão integrativa, utilizando a Biblioteca Virtual em Saúde. Os critérios utilizados para inclusão foram trabalhos disponíveis de forma completa, produções brasileiras, no idioma português, espanhol e inglês publicados entre o ano de 2015 e 2020. Foram critérios de exclusão artigos duplicados e que a temática não estivesse relacionada ao interesse da pesquisa. Síntese dos dados: percebe-se que a educação interprofissional está em consonância com os princípios e diretrizes do Sistema Único de Saúde e vem sendo incorporada no Brasil há alguns anos. Destaca-se a importância das políticas e Instituições de Ensino Superior para o desenvolvimento de profissionais colaborativos interprofissionalmente. Ainda, identifica-se a positividade percebida em relação à integração da interprofissionalidade no ensino, bem como também desafios para sua efetiva incorporação, tanto no ensino, quanto nos ambientes de trabalho. Conclusões: este estudo (re)afirma a importância da temática pesquisada, tanto na formação em saúde, como na atuação profissional, contribuindo, sobremaneira, para a melhoria da atenção à saúde no contexto dos serviços de saúde, atualmente.

Pemphigus is a group of blistering autoimmune diseases causing painful skin lesions, characterized by acantholysis and by the production of autoantibodies against, mainly, adhesion proteins. We reviewed the literature for molecules and/ or features involved in the 12 cell death pathways described by Nomenclature Committee on Cell Death, taking place in pemphigus patients, cell lines, or human skin organ cultures treated with sera or IgG from pemphigus patients or in pemphigus mouse models, and found 61 studies mentioning 97 molecules involved in cell death pathways. Among the molecules, most investigated were pleiotropic molecules such as TNF and CASP3, followed by FASL and CASP8, and then by FAS, BAX, BCL2, and TP53, all involved in more than one pathway but interpreted to function only within apoptosis. Most of these previous investigations focused only on apoptosis, but four recent studies, using TUNEL assays and/or electron microscopy, disqualified this pathway as a previous event of acantholysis. For PV, apoptolysis was suggested as a cell death mechanism based on pathogenic autoantibodies diversity, mitochondrial dysfunction, and p38 MAPK signaling. To answer those many questions that remain on cell death and pemphigus, we propose well-controlled, statistically relevant investigations on pemphigus and cell death pathways besides apoptosis, to overcome the challenges of understanding the etiopathology of pemphigus diseases.

The alpine area of the Australian mainland is highly sensitive to climate and environmental change, and potentially vulnerable to ecosystem tipping points. Over the next two decades the Australian alpine region is predicted to experience temperature increases of at least 1 °C, coupled with a substantial decrease in snow cover. Extending the short instrumental record in these regions is imperative to put future change into context, and potentially provide analogues of warming. We reconstructed past temperatures, using a lipid biomarker palaeothermometer technique and mercury flux changes for the past 3500 years from the sediments of Club Lake, a high-altitude alpine tarn in the Snowy Mountains, southeastern Australia. Using a multi-proxy framework, including pollen and charcoal analyses, high-resolution geochemistry, and ancient microbial community composition, supported by high-resolution 210Pb and AMS 14C dating, we investigated local and regional ecological and environmental changes occurring in response to changes in temperature. We find the region experienced a general warming trend over the last 3500 years, with a pronounced climate anomaly occurring between 1000 and 1600 cal yrs. BP. Shifts in vegetation took place during this warm period, characterised by a decline in alpine species and an increase in open woodland taxa which co-occurred with an increase in regional fire activity. Given the narrow altitudinal band of Australian alpine vegetation, any future warming has the potential to result in the extinction of alpine species, including several endemic to the area, as treelines are driven to higher elevations. These findings suggest ongoing conservation efforts will be needed to protect the vulnerable alpine environments from the combined threats of climate changes, fire and invasive species.

The impacts of human-induced environmental change that characterize the Anthropocene are not felt equally across the globe. In the tropics, the potential for the sudden collapse of ecosystems in response to multiple interacting pressures has been of increasing concern in ecological and conservation research. The tropical ecosystems of Papua New Guinea are areas of diverse rainforest flora and fauna, inhabited by human populations that are equally diverse, both culturally and linguistically. These people and the ecosystems they rely on are being put under increasing pressure from mineral resource extraction, population growth, land clearing, invasive species, and novel pollutants. This study details the last ∼90 y of impacts on ecosystem dynamics in one of the most biologically diverse, yet poorly understood, tropical wetland ecosystems of the region. The lake is listed as a Ramsar wetland of international importance, yet, since initial European contact in the 1930s and the opening of mineral resource extraction facilities in the 1990s, there has been a dramatic increase in deforestation and an influx of people to the area. Using multiproxy paleoenvironmental records from lake sediments, we show how these anthropogenic impacts have transformed Lake Kutubu. The recent collapse of algal communities represents an ecological tipping point that is likely to have ongoing repercussions for this important wetland's ecosystems. We argue that the incorporation of an adequate historical perspective into models for wetland management and conservation is critical in understanding how to mitigate the impacts of ecological catastrophes such as biodiversity loss.

Despite Australia's high reliance on coal for electricity generation, no study has addressed the extent to which mercury (Hg) deposition has increased since the commissioning of coal-fired power plants. We present stratigraphic data from lake sediments in the Hunter Valley (New South Wales) and Latrobe Valley (Victoria), where a significant proportion of Australia's electricity is generated via coal combustion. Mercury deposition in lake sediments increased in the 1970s with the commissioning of coal-fired power plants, by a factor of 2.9-times in sediments of Lake Glenbawn (Hunter Valley) and 14-times in Traralgon Reservoir (Latrobe Valley). Sediments deposited after the commissioning of power plants have distinct Hg isotope compositions, similar to those of combusted coals. Mercury emission, estimated using an atmospheric model (CALPUFF), was higher in the Latrobe Valley than in the Hunter Valley. This is a result of higher Hg concentrations in lignite coal, lax regulation and older pollution-control technologies adopted by coal-fired power plants in the Latrobe Valley. Near-source deposition of Hg in Australia is significantly higher than North America and Europe, where better emission controls (e.g. wet flue gas desulfurization) have been in effect for decades. The challenge for Australia in years to come will be to ratify the Minamata Convention and develop better regulation policies to reduce Hg emissions.

BACKGROUND: Individuals with type 1 diabetes (T1D) are exposed to an elevated risk of automobile accidents especially because of hypoglycemia that impairs physiological and defense responses. OBJECTIVES: To assess local risk factors for traffic events in T1D adult Brazilian patients. METHODS: This is a prospective study and 12-month follow-up to assess predictors for traffic events on a cohort of drivers with T1D (n = 168) in Brazil. The inclusion criteria for participants were Brazilian nationality, age ≥ 18 years-old, diagnosis of T1D for more than one year, driving license B, C or D categories (four-wheel vehicles), driving three-times per week or more, and checking blood glucose twice-daily or more. The primary outcome was hypoglycemia driving mishaps assessed by a seven-query questionnaire about the past 30 days. Secondary outcomes included driving mishaps not related to hypoglycemia. Statistical analysis was performed through Poisson regression models with robust variance estimarion, in which the measure of association is the relative risk. RESULTS: A total of 109 participants completed the 12-month follow-up. Most of them were men (66%) and 37 ± 11 years-old, and had a mean HbA1c of 8.2% (66 mmol/mol). In the follow up, the incidence of traffic events was high (70.6%); however, only a minority was attributed to hypoglycemia as the cause of the reported event (19.3%). The best predictors for new traffic events due to hypoglycemia were those related to driving characteristics. The best of them was a history of episodes of hypoglycemia while driving [RR 3.40 (1.22-9.43); p < 0.05]. CONCLUSIONS: We found that previous episodes of hypoglycemia while driving significantly increase the risk of new traffic events and are the best predictor for it. This highlights the need to assess the risks of traffic accidents especially in people who have had experienced episodes of hypoglycemia while driving.

There is currently a significant gap in knowledge about the emission and deposition of mercury (Hg) from coal-fired power plants in Australia. To fill this gap, we propose a novel method that combines several sources of information (stratigraphic data, hydrodynamic modelling and atmospheric modelling), to identify the sources and fates of Hg emitted from coal-fired power plants. The stratigraphic record from Lake Macquarie (Australia) shows that mercury deposition increased up to 7-times since the 1950s, which is when coal-fired power plants were commissioned in the catchment. The stratigraphy also shows a decrease in Hg deposition with power plant retrofits. Using results from multiple models (statistical modelling, hydrodynamic modelling, particle density modelling and atmospheric emissions modelling), we found that ash dams contribute little Hg to Lake Macquarie. Instead, most of the Hg contamination in the lake is a result of atmospheric emissions from the power plants, and these power plants are also depositing Hg in the urban areas to the west of the lake. Our results demonstrate that the multi-proxy approach demonstrated in the paper can be used to provide clues as to the source of Hg, so that appropriate mitigation strategies and regulatory frameworks can be implemented.

This study investigated the effect of prostaglandin E1 (PGE-1) treatment on the biochemical and histopathological changes in a model of nephropathy that was induced using renal microembolism in rats. Wistar rats were assigned to three groups: a control group (C, normal), a renal microembolism (RM) group, and a renal microembolism treated with PGE-1 (RM + PGE-1) group. The renal microembolism was induced by an arterial injection of polymethylmethacrylate microbeads into the remaining kidney of nephrectomized rats. Intramuscular treatment with PGE-1 was initiated on the day of the induction of the renal microembolism and continued once weekly for up to 60 days. At the end of the treatment period, blood samples were taken to assess the serum creatinine and urea concentrations, and 24-h urine samples were collected to determine the total protein levels. The rats' kidneys were removed and processed for histopathological analysis using the hematoxylin and eosin, periodic acid-Schiff, Mallory-Azan, and Picro-Sirius techniques. An immunohistochemical assay with vascular endothelial growth factor receptor-2 (anti-VEGFR-2) was also performed. The results showed that the PGE-1 treatment prevented vascular, glomerular, tubular, and interstitial alterations and reduced the biochemical changes, thus improving the renal function in rats that were subjected to renal microembolism. These effects could be partially attributable to an increase in the PGE-1-induced angiogenesis, because we observed an increase in the tissue expression of VEGFR-2, a specific marker of angiogenesis.

Anethole (AN) is a natural compound that has attracted great scientific interest because of its numerous biological activities, including anti-inflammatory effects. However, these effects were obtained with high doses of AN, which may be one limitation of its therapeutic use. This study evaluated the effects of a low-dose AN and ibuprofen (IB) combination on inflammatory parameters in Freund's complete adjuvant-induced arthritis (AIA) and arthritis-induced hepatic metabolic changes. Holtzman rats were used and divided into groups: normal, AIA (control), arthritics treated with IB, arthritics treated with AN, and arthritics treated with AN + IB. The volume of the paws, the appearance of secondary lesions, and the number of synovial leukocytes were evaluated. Gluconeogenesis and ureagenesis from alanine were determined in the rat liver in isolated perfusion. The AN + IB (62.5 + 8.75 mg/kg) treatment exerted an inhibitory effect on inflammatory parameters and partially prevented hepatic metabolic changes that was similar to the effect of high-dose IB (35 mg/kg) and AN (250 mg/kg) treatment. This effect of the treatments on hepatic metabolism can be, partly at least, explained by the preservation of both the alanine aminotransferase (ALT) activity and the cytosolic NADH/NAD+ redox potential in the liver. Taken together, the data obtained provided evidence that the AN + IB combination at lower doses than AN and IB treatment alone had beneficial inhibitory potential for the treatment of AIA and attenuated metabolic changes in the liver. The Hunter and Latrobe Valleys have two of the richest coal deposits in Australia. They also host the largest coal-fired power stations in the country. We reconstructed metal deposition records in lake sediments in the Hunter and Latrobe Valleys to determine if metal deposition in freshwater lakes have increased in the region. The current regulatory arrangement applied to metal emissions from coal-fired power stations in Australia are presented, discussing their capacity to address future increases in metal deposition from these sources. Sediment records of spheroidal carbonaceous particles (SCPs), a component of fly-ash, were also used as an additional line of evidence to identify the contribution of industrial activities related to electricity generation to metal deposition in regions surrounding open-cut coal mines and coal-fired power stations. Sediment metal concentrations and SCP counts in the sedimentary records, from the Hunter and Latrobe Valleys, both indicated that open-cut coal mining and the subsequent combustion of coal in power stations has most likely resulted in an increase in atmospheric deposition of metals in the local region. In particular, the metalloids As and Se showed the greatest enrichment compared to before coal mining commenced. Although the introduction of bag filters at Liddell Power Station and the decommissioning of Hazelwood Power Station appear to have resulted in a decrease of metal deposition in nearby lakes, overall metal deposition in the environment is still increasing. The challenge for the years to come will be to develop better regulation policies and tools that will contribute to reduce metal emissions in these major electricity production centres in Australia.

Historical records of mercury (Hg) deposition in lake sediments have commonly been used to monitor historic atmospheric concentrations. In the Australian environment, however, freshwater lakes are limited, restricting the region for which depositional archives of Hg can be derived. In this study we show that dendrochemistry can provide a record of atmospheric concentrations at very high resolution. We measured Hg concentrations in growth rings of two tree species from a site in western in Tasmania-Huon Pine ( Lagarostrobus franklinii) and Celery Top Pine ( Phyllocladus aspleniifolius). This region has been heavily mined over the past 150 years. Although much previous work has linked atmospheric Hg to gold mining, the evidence in this study suggests that copper smelters in Queenstown and Zeehan, not gold mining activities, were the main sources of Hg emissions to the atmosphere in this location. Huon Pine had significantly higher background Hg concentrations ( x̅ = 5.62 ng/g) than Celery Top Pine ( x̅ = 2.95 ng/g). No significant increase in Hg concentration during the peak copper smelting phase (1896 to 1935) was observed in Celery Top Pine, while a significant 1.4 fold-increase was observed in Huon Pine. Our results show that of species examined across the globe, Huon Pine is one of the most efficient bioaccumulators of Hg, making it a good proxy for tracking historical Hg emissions in western Tasmania. This ability to measure Hg in the environment is essential if Australia ratifies the Minamata Convention.

This study investigated metal contamination from historical mining in lakes in the Tasmanian Wilderness World Heritage Area (TWWHA) and surrounding region. The largest increase in sedimentation and metal contamination occurred ca. 1930 when open-cut mining commenced and new mining technology was introduced into the region. The geochemical signal of lake sediments changed from reflecting the underlying geology and lithology to that reflecting mining activities. The HYSPLIT air particle trajectory model explains metal distribution in the lakes, with those in the northwest region closest to the mines having the highest metal contamination. Lake metal concentrations since mining activities commenced are in the order: Owen Tarn > Basin Lake > Perched Lake > Lake Dove > Lake Dobson > Lake Cygnus, with Perched Lake and Lakes Dove, Dobson and Cygnus in the TWWHA. Metal contamination affected centres up to 130 km down-wind of mining sites. Enrichment factors (EF) for Pb, Cu, As and Cd are >1 for all lakes, with Owen Tarn and Basin Lake having very high EFs for Cu and Pb (98 and 91, respectively). Pb, Cu, As and Cd concentrations are above the Australia/New Zealand lower sediment guidelines, with Pb, Cu and As above the high guidelines in Owen Tarn and Basin Lake. This study demonstrated the legacy of metal contamination in the TWWHA by mining activities and the consequences of a lack of execution of environmental regulations by past governments in Tasmania.
